CVMC Membership Contract:

read, understood and agreed to the following conditions whereas my membership with CVMC is 
concerned:
1.  That my Visa/Mastercard/American Express credit card 
(circle the one that applies), number _______________________ and expiration date ________, has 
been charged the $34.99 membership fee and will be charged for any films I decide to rent in the 
future plus the shipping and handling fee;
2.  That the aforementioned bankcard will be charged for any film I intentionally or 
unintentionally destroy (excluding damaged during shipment) or fail to return*, or else my 
membership with CVMC will be revoked without refund;
3.  That the aforementioned bankcard will be charged $4 per film per day for late fees, or else 
my membership with CVMC will be revoked without refund.

*       Film prices vary greatly, can be anywhere from $15 to $180; a general estimate for films 
in CVMC?s collection is $50 per film.  You will be charged the retail price for any film that is 
not returned or which is damaged.
